SAGE Instrument
The WHO-SAGE India instrument has following Schedules:
1. Household Schedule
0400 Household Roster :
- Create a list of household occupants from which a respondent will be randomly selected;
- Better understand the impact of the household members on household dynamics and individual health;
- Collect basic information required for estimating the non- response bias in the survey; and,
- Provide key information for statistical analysis in order to determine the health-related outcome rates in respective countries.
0500 HOUSING
- Dwelling/Home
- Environmental Risk Factors/Water and Sanitation
0700 Assets and Household Income & Assets:
- Record the number of items the household possesses, regardless of their condition
- Asks about the total income for the household in the last 12 months from paid work or all other sources

- The section covers the expenditure in different time periods:
- Last 30 days and Last 12 months
2. 0910 Verbal Autopsy
- SAGE Wave 3 has used the 2012 version of the WHO VAQ. The same version used during SAGE Wave 2.
- SAGE has opted to use the VAQ instrument for death of person in the household aged 50 years or more at the time of death
- Verbal Autopsy-2012 version has 192 cause of death related indicators, subdivided into 4 sections and 118 subgroups.
3. Individual Schedule
1000 Socio-Demographic Characteristics :
- This section provides information on different background characteristics and gives an indication of income and socio-economic status of respondents.
2000 Health State Description:
> This section asks questions about overall health. It covers all aspects of physical and mental health, including:
- Mobility
- Self-care
- Pain and Discomfort
- Cognition
- Interpersonal Activities
- Sleep and Energy
- Affect
- Hearing
- Vision
- Functioning Assessment
3000 Risk Factors and Preventive Health Behaviours:
This section on risk factors and preventive health behaviours relate to some aspects of how we live on a daily basis.
In particular, they are designed to record details about:
- Tobacco use (Smoke and Smokeless)
- Alcohol consumption
- Nutrition, and
- Physical activity
4000. Chronic Conditions and Health Services Coverage
Assesses chronic conditions (through self-reporting and symptoms) and access to health care services.
Evaluates access of the poor to effective interventions. The questions are about:
- Diagnosed conditions
- Treated conditions,
- Plus common symptoms for these conditions
5000 Health Care Utilization:
Know about the recent experience with obtaining health care from health care worker, hospitals, clinics, and the heath care system. Questions are asked about:
– Needing health care
– Inpatient care and Outpatient care and care at long term facilities,

Asks questions about: Social network , Feelings, Trust , Security/safety
7000-Subjective Well-being and Quality of Life:
- SAGE uses an adapted version of the Day Reconstruction Method (DRM) to assess hedonic well-being i.e. “Information about the allocation of time in the daily life and measures of the satisfaction people derive from their activities”
SAGE uses the following to capture evaluative well-being:
2 Global questions
Q7008 Taking all things together, how satisfied are you with your
life as a whole these days (from WVS-World Values Survey)
Q7010 Taking all things together, how would you say things are
these days (from GSS-General Social Survey)
- Short version 8 item WHOQoL instrument (Q7001-Q7007, Q7009)
- Cantril self-anchoring ladder
2500 Anthropometrics, Performance Tests & Biomarkers
A critical objective of incorporating a biomarker module in SAGE is to compare and measure prevalence of health conditions with self-reported prevalence.
